Chapter 14: Forms and Petitions

How to Access Forms

All academic forms required for completion of the IC-PhD programs have been pre-routed for students in GT eSignature (DocuSign).

Steps to access:

Alternatively, navigate to: Templates > Shared Folders > Students > COC > Interactive Computing

Students will need to log in using their GT ID and password before they are able to access DocuSign forms.

For all GT recipients, please use the GT DocuSign Address Book.

Form Usage and Notes
Admission to Candidacy Request for permission to PhD Candidacy. Route to Academic Program Coordinator (Theresa Nash) for SIC signature.
School of Computer Science Special Problems (CS 8903) Permit Form Allows a student to register for a specified 8903 section. Variable credit. Due: First day of classes.
Computer Science PhD Advisor Selection and Pre-Dissertation (CS 8999) Permit Form For designating or changing advisors. Variable credit. Due: First day of classes.
Computer Science CS 9000 Form Used when CS student passes the qualifier.
Computer Science PhD Breadth Component Form To show completion of the CS PhD Breadth component.
Computer Science PhD Minor Form To show completion of the non-CoC coursework which comprises the Institute's PhD Minor component. 2 courses (6 credits), no less than B grades, with more As than Bs.
Human Centered Computing PhD Minor Form To show completion of the non-CoC coursework which comprises the Institute's PhD Minor component. 2 courses (6 credits), no less than B grades, with more As than Bs. Due: One week prior to the beginning of Phase I registration.
Human Centered Computing PhD Qualifier Form To show completion of the HCC PhD Qualifier Exam. Due: One week prior to the beginning of Phase I registration.
Human Centered Computing (CS 9000) Permit Form For approval to register in post-qualifier PhD dissertation research hours. Variable credit.
Human Centered Computing PhD Specialization Permit Form HCC students use this form to confirm successful completion of the Core and Specialization requirements of their program.
Robotics PhD Qualifier and 9000 Permit To show completion of the Robotics PhD Qualifier Exam and to obtain permit for CS 9000.
PhD Thesis Forms Completed after students have submitted their dissertation. Route to the Academic Program manager (Theresa Nash) or Associate Chair (Thomas Ploetz) for signature.
Graduate Student Exit Survey To be completed by graduate students in the MS CS, MS InfoSec and CS PhD programs in conjunction with the petition to graduate. Due: Prior to the deadline for graduation petitions.
Employment Certification Request For certification request of employment in the College of Computing. Due: Please allow two business days for processing.
Student Request for Travel Funds Request reimbursement of travel expenses for a conference, workshop or other sponsored activity. Limited funds available; request as soon as need is known (e.g., paper acceptance). Due: Normally prior to making reservations; no later than one week prior.

Submit via DocuSign. Send it to the graduate program coordinator: Theresa Nash.

Signature Authority: The Board of Regents of the University System of Georgia (BOR), in its policy manual, specifically addresses the issue of authorization to enter into contracts on behalf of the BOR.
