How to Access Forms

All academic forms required for completion of the IC-PhD programs have been pre-routed for students in GT eSignature (DocuSign).

Steps to access:

• Log in to GT eSignature

• Under the 'Template' tab, search 'coc-ic' to pull up forms

• This will bring up all IC forms

• Forms have been pre-routed for your convenience

• Add advisors where necessary

Alternatively, navigate to: Templates > Shared Folders > Students > COC > Interactive Computing

Students will need to log in using their GT ID and password before they are able to access DocuSign forms.

For all GT recipients, please use the GT DocuSign Address Book.
